Health is a state of physical, mental and social well-being in which disease and infirmity are absent.
If the 40 years of fasting that they missed was with a valid excuse, then all they need to do is pay a 'fidya' for each day. A fidyah is 3/4 of a kilo of food, given to poor.
If they did not have an excuse, then they must:
1. Repent to Allah ta'ala.
2. Pay kaffarah for each day. Kaffarah is feeding of 60 poor people (most common option).
3. Write in their will that they have this amount of qadha' fasting to be done once they die, either by their next of kin, or they pay someone who will fast as qadha` on their behalf.
Furthermore, from this holy month of Ramadhan, because they cannot fast, they can pay the fidyah.
And Allah knows best.

Yes, it is permissible as far as there is no strong possibility of risk on their life. They need to seek scientific experts' advice.
There is a supplication from Imam Zayn al-'Abidin (a) related to ill health.
His Supplication when Sick or Visited by Distress or an Affliction
1- O God, to Thee belongs praise
for the good health of my body
which lets me move about,
and to Thee belongs praise,
for the ailments
which Thou causest to arise in my flesh!
2- For I know not, my God,
which of the two states deserves more my thanking Thee
and which of the two times is more worthy for my praise of Thee:
3- the time of health,
within which Thou makest me delight in the agreeable things of Thy provision,
through which Thou givest me the joy to seek
the means to Thy good pleasure and bounty,
and by which Thou strengthenest me
for the acts of obedience
which Thou hast given me success to accomplish;
4- or the time of illness
through which Thou puttest me to the test
and bestowest upon me favours:
lightening of the offenses
that weigh down my back,
purification of the evil deeds
into which I have plunged,
incitement to reach for repentance,
reminder of the erasure of misdeeds through ancient favour;
5- and, through all that, what the two writers write for me:
which no heart had thought,
no tongue had uttered,
and no limb had undertaken,
rather, as Thy bestowal of bounty upon me
and the beneficence of Thy benefaction toward me.1
6- O God,
bless Muhammad and his Household,
make me love what Thou hast approved for me,
make easy for me what Thou hast sent down upon me,
purify me of the defilement of what I have sent ahead,
erase the evil of what I have done beforehand,
let me find the sweetness of well-being,
let me taste the coolness of safety,
and appoint for me
a way out from my illness to Thy pardon,
transformation of my infirmity into Thy forbearance,
escape from my distress to Thy refreshment,
and safety from this hardship in Thy relief!
7- Thou art gratuitously bountiful in beneficence,
ever gracious in kindness,
the Generous, the Giver,
Possessor of majesty and munificence!

there are three major fatwas regarding smoking tobacco and cigarettes:
1) It is haram.
2) It is allowed provided that it would not cause substantial harm to your health.
3) It is haram to start but if you are already an habitual smoker it is allowed but you should try to stop, even gradually.
Overall, it is at least definitely advisable not to smoke.

With self-reformation and purification, a person becomes enlightened and is adorned with beautiful character traits. Their source of strength is God and they are far away from worshipping their own egos. They are a source of blessing and peace to all those around them. Since the struggle with the self is one of the most important aspects of the spiritual message of Islam, mental health is something that is extremely important. It is unfortunate to see many Muslims chasing after mental health solutions in traditions that are sometimes even secular in their outlook which focus on the strengthening of the ego rather than humbleness in front of God, as that is usually due to a lack of familiarity with the solutions that are offered within Islam.

There are many books compiled on medical narrations on health and food from the Prophet (SAWA) like Tibb al-Nabi or Al-Tibb Al-Nabawi and from Ahlul Bayt (AS) like Tabb Al-A'yimmah or Tibb Al-Sadiq (AS).
Sayyed Mahmood Dehsurkhi who is a well known scholar in Hadeeth (passed away recently in Qum) has a books in this regard called Ramz Al-Sihhah Fi Tibb Al-Nabi wal A'iymmah. He has collected in 277 pages good number of Hadeeths in health and food. The book was published by Lahoot publications tel. +98-251-7747493.

One should see a specialist and doctor (preferably Muslim) to determine whether fasting could be truly harmful for them after recovering. If it was ruled to be harmful and you yourself know it that can harm you, then it’s possible for you to be exempted until your state improves.

Mental health is part of the well being of the soul, and in Islam a person who with good mental health is one who benefits from the realities of faith and wisdom. Through their relationship of trust in God and faith in Him, His Wisdom and His Kindness, there is nothing from the transient world that can break him/her. It is through their relationship with God that they find peace and solace, hope and tranquillity. Their hearts are enlivened and their minds are active. Their actions are measured with the insight into both worlds and their lives are beautiful and successful due to their attitudes and actions.
The well being of the soul is discussed extensively in the scriptural sources (Qur'an and hadith) and is also discussed in the fields of ethics, philosophy and theoretical and practical mysticism.

Simply, start after the Month of Ramadhan making up (Qadha) of all the days which you missed and you must also pay Kaffara of ever day you deliberately missed by feeding 60 poor believers for every day of missed fast.
Yes, it is true that the Prophet (SAWA) and Infallible Imams used to worship Allah for very long hours in the nights ( two thirds or half or one third of the night) and in special nights for the whole night.
We are not supposed to spend the whole night in worshipping and leave our responsibilities in studying and working , but need to spend some time in the night in worshipping Allah (SWT), thanking His Bounties and preparing ourselves for our real future (Aakhirah).
Thank you for your question. They (as) spent the whole of specific nights in worship, but slept in the night on a regular basis. Part of their night was always spent in prayer. A small amount of praying per night should not negatively effect most people's health.
