If the husband is totally impotent and unable to penetrate, then his wife has the right to seek divorce through the Haakim Al-Sharee'ah who is the Marji' of Taqleed who gives the husband one year to treat himself. After one year, if husband is still unable to penetrate, then the Marje' of Taqleed issues an order of Faskh which mean nullifying the marriage.

If the ejaculation occurred without intending to do that, Qadha is must of that fast. Kaffara is obligatory if it was done intentionally. But how can one masturbate with out intending to do it? Was he sleeping. If he masturbates while he was awake then he has committed it intentionally. If he masturbated but his intention was not to reach to ejaculation, and ejaculation occurred with out his control, that might be possible.

Yes, from a shar'i perspective, if no lust or inappropriate comments or gestures are made about someone, you can say "so and so is handsome/beautiful". Is it a good idea, I don't think so. One's physical appearance should not be the point of conversation between people, especially if they are non-mahram and opposite gender. 

It could be used against you, or taken out of context, or understood with other intentions.

If both of them were really exempted from fasting due to illness which prevents fasting or due to traveling, they will then be allowed to engage in intimacy during the day of month of Ramadan.
