Ghusl

Ghusl (Arabic: غسل‎) is an Arabic term referring to the full-body ritual purification mandatory before the performance of various rituals and prayers, for any adult Muslim after having sexual intercourse, ejaculation or completion of the menstrual cycle, although wudu or just sleep, without either, are the lesser stages of purity that is acceptable.The washing is also recommended but not required (i.e. it is mustahabb) before Jumu'ah and Eid prayers, before entering the ihram in preparation for Hajj, after having lost consciousness and after formally converting.

As far as you had the intention of Ghusl and you made water reach to your head then your body’s right and left side, your Ghusl is valid.

Yes, it is permissible for her to wear it, as long as the script of the Quran does not touch her skin if she does not have wudhu'. To avoid this, she can wear it on top of clothing, or keep the script to the front, or cover it with a thin transparent layer of nail-polish, or something like that, which can also be used as a protection for it as well.

Of course, she must also ensure that it stays away from any najasah.

Every obligatory Prayer is obligatory on you when you have Istehagha blood but you need to perform for it either one Wudhu for every Prayer if the quantity of blood is little which does not fill the pad, or add with one Ghusl before Fajr Prayer if the blood fills the pad but does not leak out of it (Medium Istihadha) or perform three Ghusls daily; one before Fajr Prayer, then one before Dhuhr Prayer then one before Maghrib Prayer, when the bleeding fills the pad and leak out if it (Big Istehadha).

There is no special Prayer named as Istihadha Prayer., but if you missed any Prayer during Istehadha, you need then to perform it as Qadha like any obligatory missed Prayer.

No. Henna colour is just colour which does not form a layer preventing water from reach to your skin hence you don't need to remove such colour for performing Ghusl. You must perform Ghusl immediately after your menstruation is over to perform your obligatory Prayers.

Looking at this example, the reason to avoid ghusl is for aesthetic reasons, so as to avoid loss of hair, due to excessive ghusls per day.

The couple can cut down on what leads to ghusl, and prioritise their 'ibadah and make sure their relationship with Almighty God is always stronger than their lust. 

Intimacy is very good, but not such that it makes one think in this kind of a way, and deliberately miss out on prayer. 

Ghusl should not take more than 2 minutes, all together, and not that much water as well. Therefore, if the couple do have a fear of hair loss, minimising the shower time is good option. It is also mentioned that cold water is good for hair.

Usually, the capacity of water tanks now a days is much more than Kurr unless you know that it is a very small tank. If you doubt whether the quantity of the water is Kurr or less, you need then an evidence to deal with it as Kurr.

It is not necessary to pour water during Ghusl three times. The obligatory act is to make the water reach to your head, face and whole body one time. Repeating that does not harm the Ghusl.

You need to be sure that the menstruation cycle has ended and there is no blood at all, then perform Ghusl of Haydh. If you doubt that the menstruation might not finished yet, you need to wait and check till you become sure that menstruation has really ended.

Minimum period of menstruation is three days continuously and maximum period of menstruation is ten continuous days. Bleeding after ten days is not menstruation but Estihadha during which SALAH is obligatory with rules of Wudhu or Ghusl.

Being under the water for 5 minutes would, in most, if not all cases make water reach all parts of your head, and that would be enough for your ghusl. The important thing is you follow the correct procedure of ghusl, which is head/neck, then right side of body, then left side of body.

Avoiding excessive doubt is also very important.
