Salah ("worship", ‏صلاة‎; pl. ‏صلوات‎ ṣalawāt; also salat), or namāz (Persian: نَماز‎) in some languages, is one of the Five Pillars in the faith of Islam and an obligatory religious duty for every Muslim (Male & Female, with certain exemptions based on Taqwa). It is a physical, mental, and spiritual act of worship that is observed five times every day at prescribed times.

You need to refer to the experts in your area who have authentic evidence on the time of  Fajr.

If you can not find such trusted experts,bayou should practice precaution (Ehtiyat).

We don't pray any obligatory prayer before being sure that the time has started.

No. There is no obligation to wear any cap while performing SALAH or reciting Quran. Women should obviously wear Hijab but there is no obligation on men to wear a cap or so. It is recommended for men who can to wear turban to do so while praying SALAH. No obligation at all to cover the head or wear a skull cap.

Yawning does not invalidate the ritual prayers (salat), although it is better manners to avoid excessive yawning if possible.

If you are very sleepy and have time to take a nap before the salat time ends, you can do that too. 

There are some hadith narrated from Abu Hurayra along the lines that yawning and sneezing are from Shaytan. Some Sunni Muslims accept these hadith. Shi'i Muslims usually reject these hadith as incorrect/untrue (that is, in the Shi'i view, yawning and sneezing do not come from Shaytan). You may select whatever viewpoint is suitable for you (and Allah knows best).

Asr Prayer’s time starts immediately after completing Dhuhr Prayers, but if you want to separate between Dhuhr and Asr, then when the shadow of an item reaches two sevenths of it. 
Isha prayer can be performed after performing Maghrib Prayer and it’s Nawafil but if you want to separate between Maghrib and Ish, then after the end of reddishness on the west.

In the incident of you missing out on your prayer intentionally, repent for that, if you have not already done so, do the prayer you missed out on (as qadha`), and focus more on prioritising your prayers, in shaa Allah

Being under the water for 5 minutes would, in most, if not all cases make water reach all parts of your head, and that would be enough for your ghusl. The important thing is you follow the correct procedure of ghusl, which is head/neck, then right side of body, then left side of body.

Avoiding excessive doubt is also very important.

If you recite Sura Al-Dhuha in your Prayer, you must recite along with it Sura Al-Inshirah because these two Suras are considered as one Sura in the Prayer. You must recite Bismillah Arrahman Arraheem for every Sura of them.

Same rule is for reciting Sura Quraish and Sura Al-Feel after Sura Al-Hamd in the Prayer.

Muslims are allowed to join Dhuhr and Assr means to pray Assr after completing the Dhuhr prayer. Also to pray Isha after completing Maghrib prayer. This is according to the authentic Prophetic Hadeeths that the Prophet Muhammad (SAWA) used to join his Prayers in Madina to avoid putting burden on his Ummah.

‘Muslims are allowed to join whenever and where ever they want.

‘Joining the Prayers does not mean to stand immediately for Assr after finishing the Dhuhr prayer. We recite Quranic verses and supplications after every Prayer, then we go for the next prayer.

No, there is no situation where a person is allowed to not pray. 

If they forget, or if they are sleeping, this will be an exemption from sin, and they can do their prayer that they missed out because they were sleeping or forgot.

Otherwise, as long as you are awake and know its prayer time, you must pray, whatever the circumstance may be. (of course  a female if she is in her haydh or nifas, she does not pray).

If you have missed out on prayers, intentionally, then you must repent and do those prayers as qadha.

What we can see from our esteemed scholars is they refer to this prayer as "salat laylatul-dafn", the prayer of the night after burial, or "salat hadiyatul mayyit" prayer of a gift to the deceased, or "salat laylatul wahshah", prayer of the night of loneliness.

One narration for this prayer is: 

قال رسول الله (ص) لا يأتي على الميت أشد من أول ليلة فارحموا موتاكم بالصدقة فإن لم تجدوا فليصل أحدكم ركعتين: يقرأ في الأولى الحمد وآية الكرسي وفي الثانية الحمد والقدر عشرا فإذا سلم قال: “اللهم صل على محمد وآل محمد، وابعث ثوابهما إلى قبر فلان”. فإنه تعالى يبعث من ساعته ألف ملك إلى قبره مع كل ملك ثوب وحلة …

This is in al-Misbah, by al-Kaf'ami, p. 164.

Our Maraji' taqleed also consider this prayer to be mustahab as well.

Booking driving lesson or anything of this sort is not at all a valid excuse for missing out on your wajib prayers. Your parents should have arranged it in such a way where you can pray, and the duty of parents is to teach their children to always prioritise prayer. 

Yes, if you will be late for prayers, and will still be able to pray within the time frame for prayer, that would be permissible, although its always best to pray on time. So, it would be ok for you to go to the driving class, as long as you have time to pray.

However, if you will not have enough time, then it will be impermissible and haram.
