If I bought a phone SIM with Haram money a while back, and I now do online shopping with my legitimate earnings, does the fact that a verification code is sent to that phone render such purchases disallowed?

Haraam money is always Haraam till you give it back to the real owner. You are not allowed to use the SIM which you have purchased with Haraam money till you pay that money back to the real owner or his inheritors.

Is it allowed to buy from a shop that is playing music? Are we obligated to ask them to stop playing such music before we can buy from them?

Listening to sinful music is a sin. We should try to avoid shops and
places where music is being played. If there is no other alternative
and if we have to buy from a specific shop which is playing music, it
is good to request them to switch off the music if there is a possibility
that the shopkeeper will listen to your request. If they don’t listen
to your request, we have to be very quick to get our work done and
move away as soon as possible
