If he knows that his masturbating can lead him to ejaculation by accident, his act of masturbation is a sinful act and he has to pay Kaffara along with Qadha of the fast if that day.

If ejaculation happens completely unintentionally then no Kaffara.

It is not permissible for the husband (according to obligatory precaution) to drink from the milk of his wife. Such act despite being not permissible, does not forbid the husband from sexual activities with his wife nor does it harm their marriage bond. If husband while his sexual activities with his wife gets some milk in his mouth, he should spit it out and avoid drinking it.

Semen does not gush out without your feeling it. Stain can be from other fluid which is not semen. Your fast is valid as far as you did not feel the orgasm of the ejaculation and caused it deliberately.

Quranic verse are very clear in ordering the believers not to mention any thing which can hurt the feelings of a believer or might cause disrespect to him. Discussing marital intimacy or seeking clarification about one's spouse must not cause any harm or disrespect or annoying the spouse in any way.

Bismihi ta'ala

There is no kaffarah or anything to be paid, but there are two very important thing everyone must always remember.

1. Parents, or anyone being intimate must secure their privacy first, making sure there will be no intrusion, and have absolute certainty that nobody can or will be able to see or hear them. Even infant children should be away from their parents, and we have narrations from Ahlul Bayt (a.s.) that strongly censure parents being intimate with the presence of even a baby. 

2. Whenever someone is living with other people, he/she must always remember to announce themselves, knock doors, make a noise, say "Ya Allah", to inform others of their presence, and so on. This is to avoid seeing somebody without their clothes, or in a situation that they are uncomfortable in, etc. 

In the case something happens unintentionally, it should be forgotten, not mentioned, and just be stricter in observing these two points.

If you are sure that the fluid is a semen then Ghusle Janabat is obligatory but some white fluid can come out after urinating which looks like semen but it is not semen but called Wathi وذي , it does not need Ghusle Janabat.

If you did not know that masturbation invalidates the fast and you committed masturbation during the days of the month of Ramadan there is no Kaffara on you but Qadha of the fast of that day. If you did not know that Ghusl of Janabah is compulsory for a valid fasting you need to keep Qadha (Qaza) for every day of fasting which you are sure that you were in the state of janabat. Wassalam.

If you sincerely decide to normalize your sexual orientation, you will be able to do that with help of Allah. It depends on you and your firm decisions. There is a narration from Imam Al-Sadiq (AS): Human being will never be weak to ache if what his strong will wants.

'It is good to remember the sexual orientation  of all the prophets and Imams and their followers and terrible end of those who followed wrong sexual desires as we read in Quran.

'It might be also useful to consult psychiatrists in some cases.

'You also need to avoid mixing with persons who have wrong sexual orientations.
