Ghusl (Arabic: غسل‎) is an Arabic term referring to the full-body ritual purification mandatory before the performance of various rituals and prayers, for any adult Muslim after having sexual intercourse, ejaculation or completion of the menstrual cycle, although wudu or just sleep, without either, are the lesser stages of purity that is acceptable.The washing is also recommended but not required (i.e. it is mustahabb) before Jumu'ah and Eid prayers, before entering the ihram in preparation for Hajj, after having lost consciousness and after formally converting.

During Ghusl, is it necessary to pour water over the head three times? If I feel that the water hasn't reached my scalp properly, can I wash it more than three times, or would that invalidate the Ghusl, requiring me to start over?

It is not necessary to pour water during Ghusl three times. The obligatory act is to make the water reach to your head, face and whole body one time. Repeating that does not harm the Ghusl.

You need to be sure that the menstruation cycle has ended and there is no blood at all, then perform Ghusl of Haydh. If you doubt that the menstruation might not finished yet, you need to wait and check till you become sure that menstruation has really ended.

Minimum period of menstruation is three days continuously and maximum period of menstruation is ten continuous days. Bleeding after ten days is not menstruation but Estihadha during which SALAH is obligatory with rules of Wudhu or Ghusl.

Being under the water for 5 minutes would, in most, if not all cases make water reach all parts of your head, and that would be enough for your ghusl. The important thing is you follow the correct procedure of ghusl, which is head/neck, then right side of body, then left side of body.

Avoiding excessive doubt is also very important.

We must try our best to perform every Prayer within its time and never miss any Prayer as much as we can. If God firbid, Fajr Namaz is missed, we must perform its Qaza as soon as we can. There is no condition to perform Ghusl of Jum’a before performing the Qaza although Ghusl Jum’a is very great recommended Ghusl, but you can perform the Qaza of Fajr with Wudhu only or after Ghusl Jum’a if you wish but Ghusl Jum’a is not obligatory nor a condition for Qaza Namaz.

As this matter is a new matter, and the monitor prevents water from reaching to part of your skin, many of our Maraaji' of Taqleed say that you need to perform both Ghusl and Tayammum. During the Ghusl, you need to wipe over the monitor  on your body.

No, that item touched by a person who had touched a corpse and did not perform Ghusl yet, will not become Najis (impure).

Islam encourages healthy sexual relations between husband and wife, and to cater for each other's sexual needs and lusts, as long as they are within the shar'i and moral boundaries. 

It is highly mustahab to address their sexual needs. For example, foreplay is highly recomended, where first it is recommended to engage in mudāʿabah, which means verbal foreplay, then mulāʿabah, physical foreplay, and then mujāmaʿah, which is having sex. 
Oral sex is a part of mulāʿabah, and there is no problem with that as well. In regards to semen, in Shi'i fiqh, it is considered najis, and therefore, should the husband ejaculate in wife's mouth, she must not swallow any of it, and must wash her mouth. 

Also, the wife does not need to perform ghusl in this instance.

Of course, there are certain sexual practices in today's Western society that are inhumane, immoral, degrading and humiliating, but happily accepted by some. As Muslims, we we must not imitate whatever is out there, or  what is in pornography, etc.

As much as our religion encourages intimacy and sex within marriage, we also have a duty to dignify our spouse and never make her feel uncomfortable.

Semen does not come after urination but it gushes out in ejaculation. There is a white liquid which comes after ruination called Wathi وذي  which is not Najis and does not need any Ghusl.

Yes. Most of our present eminent Maraaje' say that Ghusl of Friday suffices Taharah and you can pray with it with out need for Wudhu.

If you touch any part of the dead body or bone of a dead body directly with out gloves, you need to perform Ghusl of touching a dead. If you touch wearing gloves, then you don't need to perform Ghusl of touching the dead.
